Falij (Stroke) in Pakistan: The FAST Warning Signs and Recovery

Stroke was the second leading cause of death in Pakistan in 2021, causing 99,759 deaths per Global Burden of Disease data. A sourced guide to FAST warning signs, the treatment window and rehabilitation.

Stroke is Pakistan's second leading cause of death. The Global Burden of Disease Study 2021 attributed 99,759 deaths in Pakistan to stroke in that year, with an incidence of 153 per 100,000 and a prevalence of 1,088 per 100,000 ("Epidemiology of stroke in Pakistan and its provinces, 1990–2021," ScienceDirect). The leading risk factor for stroke death was high systolic blood pressure — which is to say, the largest single cause is both measurable and treatable.

At a glance

  • 99,759 stroke deaths in Pakistan in 2021 — the second leading cause of death (GBD 2021)
  • High blood pressure is the single largest contributing risk factor
  • Clot-busting treatment works within roughly 4.5 hours of symptom onset — the window is the whole story
  • Use FAST: Face, Arms, Speech, Time
  • Recovery is driven by rehabilitation, and the first three to six months matter most

FAST — falij pehchanne ka sab se aasan tareeqa

  • F — Face (chehra): Muskurane ko kahen. Kya munh ek taraf se terha ho raha hai? (munh terha hona)
  • A — Arms (bazu): Dono bazu uthane ko kahen. Kya ek bazu neeche gir jata hai ya uth hi nahi raha?
  • S — Speech (bolna): Ek sada jumla dohrane ko kahen. Kya zaban lardkharati hai, ya baat samajh nahi aa rahi?
  • T — Time (waqt): In mein se koi bhi alamat ho to fauran 1122 par call karen. Waqt note karen — doctor sab se pehle yehi poochenge.

Other sudden symptoms that count: numbness down one side (hath pair sun hona), sudden loss or blurring of vision in one or both eyes, sudden severe headache unlike any before, sudden loss of balance or coordination, and sudden confusion.

Why the clock matters so much

Most strokes are ischaemic — a clot blocking an artery to part of the brain. Clot-dissolving treatment (thrombolysis) can restore blood flow, but only within roughly 4.5 hours of the symptoms starting, and mechanical clot retrieval within a somewhat longer window in selected cases. Brain tissue dies at a rate often quoted as around 1.9 million neurons a minute. A haemorrhagic stroke — a burst vessel, dimagh ki nas phatna — needs the opposite management, which is precisely why a CT scan comes before any treatment and why home remedies are so dangerous.

Do not wait for it to pass, give food or water (swallowing is often unsafe), give aspirin (harmful if it is a bleed), or try to "massage the numbness out". Note the time symptoms began and go.

TIA — the warning you get to act on

A transient ischaemic attack produces the same symptoms but resolves within minutes to hours. It is not a false alarm; it is a warning, and the risk of a full stroke in the days that follow is high. A TIA needs urgent assessment the same day — not a follow-up appointment next month.

Falij kyun hota hai — risk factors

  • High blood pressure — the largest single contributor, and often symptomless until this
  • Diabetes
  • Smoking, naswar and sheesha
  • Atrial fibrillation — an irregular pulse that throws clots; often undiagnosed
  • High cholesterol, obesity, physical inactivity
  • Household air pollution and ambient particulate matter — both named among Pakistan's leading contributors in GBD 2021

Recovery: falij ke baad kya hota hai

Survival is the first half; function is the second, and it is the half that rehabilitation decides. Recovery is fastest in the first three to six months, though gains continue well beyond that. What matters most is starting early and doing it consistently.

  • Physiotherapy — regaining strength, balance, standing and walking; preventing contractures and falls
  • Occupational therapy — relearning dressing, eating, washing and the specific tasks of daily life
  • Speech and language therapy — for slurred speech, word-finding difficulty (aphasia) and unsafe swallowing
  • Secondary prevention — blood pressure, blood sugar, cholesterol and often blood thinners; a second stroke is a real risk
  • Mood — depression after stroke is common, under-recognised and treatable. See depression and anxiety

A physiotherapist should be involved within days, not months. Home programmes between sessions do much of the actual work — our back pain guide explains why adherence between visits matters more than the visits themselves.

Frequently asked questions

Falij ke baad kitna theek hona mumkin hai?

Yeh is par munhasir hai ke dimagh ka kitna hissa mutasir hua aur ilaj kitni jaldi shuru hua. Bohat se log kaafi had tak behtar ho jate hain, khas taur par agar physiotherapy jald shuru ho. Pehle teen se chhe mahine sab se ahem hote hain — lekin us ke baad bhi behtari hoti rehti hai, is liye mayoos hone ki zaroorat nahi.

Kya falij dobara ho sakta hai?

Ji haan, aur yehi sab se bara khatra hai. Isi liye blood pressure, sugar aur cholesterol ka ilaj zindagi bhar jari rakhna parta hai. Dawai band karna — chahe tabiyat theek lag rahi ho — dobara falij ki sab se aam wajah hai.

Is a stroke the same as a brain haemorrhage?

Not quite. Both are strokes, but one is a blockage and the other a bleed, and their treatments are opposites — a clot-buster that saves one would kill the other. That is why a CT scan comes first, and why nobody can safely treat a stroke at home.
