Dil Ka Daura (Heart Attack) in Pakistan: Warning Signs and What To Do First

Cardiovascular deaths in Pakistan roughly doubled between 1990 and 2019 per Global Burden of Disease data. A sourced guide to heart attack symptoms, the first ten minutes, and prevention.

Heart disease has quietly become Pakistan's leading killer, and it is arriving earlier than it used to. Analysis of Global Burden of Disease data found the country's annual cardiovascular deaths rose from roughly 170,000 in 1990 to about 340,000 in 2019 — a doubling in a single generation ("Pakistan's rising cardiovascular disease burden associated with modifiable risk factors," PMC). Pakistan's age-standardised CVD incidence, 918 per 100,000, sits well above the global figure of 684.

At a glance

  • Annual CVD deaths roughly doubled between 1990 and 2019 (GBD data)
  • Ischaemic heart disease caused 53.8% of Pakistan's CVD deaths in 2019
  • Heart attacks in the 30–40 age group are increasingly common here — young does not mean safe
  • Chest pain is the classic symptom, but women, older people and those with diabetes often present without it
  • Time is muscle: the first hours decide how much heart survives. Do not "wait and see"

Dil ke daure ki alamat

Yeh alamat pehchan lena zindagi bacha sakta hai:

Atypical presentations matter. Women, people over 70, and people with diabetes — whose nerve damage blunts pain — may have no chest pain at all. Breathlessness, sudden fatigue, or what feels like indigestion can be the only sign. In Pakistan, where diabetes is extraordinarily common, this is not a rare exception.

Agar shubah ho to pehle 10 minute mein kya karen

  1. Call 1122 immediately. Ambulance mein ilaj shuru ho jata hai; apni gaari mein le jana us se behtar nahi hai — aur agar raaste mein dil band ho jaye to gaari mein koi madad nahi kar sakta.
  4. Do not wait to see if it passes. The commonest fatal mistake in Pakistan is waiting an hour to see whether it is "gas". Treatment that opens a blocked artery works best within the first 90 minutes to two hours.
  5. If the person becomes unresponsive and is not breathing normally, start CPR — hard, fast compressions in the centre of the chest, about 100–120 a minute — and keep going until help arrives.

Do not give food or drink. Do not give someone else's prescription nitrate spray. Do not drive yourself.

Why it is happening earlier here

South Asians develop coronary disease roughly a decade earlier than Western populations, at lower BMIs, and the risk factor picture in Pakistan explains much of the rest: very high rates of diabetes and high blood pressure, widespread smoking and naswar, rising obesity, low physical activity, and heavy use of ghee and fried food. Household and ambient air pollution are independently significant. Family history matters and is not modifiable — but everything above it is.

Tests you may be offered

  • ECG — minutes, cheap, and the first thing done in any emergency room for chest pain
  • Troponin blood test — detects heart muscle damage; usually repeated after a few hours
  • Echocardiogram — ultrasound of the heart's pumping and valves
  • ETT / stress test — for stable chest pain on exertion, not for an active attack
  • Angiography — a dye study that shows exactly which artery is blocked, and allows a stent in the same sitting

A normal ECG does not rule out a heart attack. If symptoms are convincing, insist on serial ECGs and troponin rather than accepting a single reassuring strip.

Prevention: dil ki hifazat kaise karen

  • Blood pressure — know your number; treat it. Hypertension is the single largest contributor to CVD deaths here
  • Blood sugar and HbA1c — screen from 30 in South Asians, earlier with family history
  • Cholesterol — a fasting lipid profile; LDL is the number that drives treatment
  • Stop smoking, naswar, gutka and sheesha — risk starts falling within weeks
  • 150 minutes of brisk walking a week
  • Cut ghee, fried food and salt; more vegetables, daal and fibre
  • Treat sleep apnoea if you snore heavily and wake unrefreshed

When to see a doctor

Emergency now for the symptoms above. Non-urgently, see a cardiologist if you get chest tightness or breathlessness on exertion that eases with rest, if you have diabetes or hypertension and have never had a cardiac assessment, or if a parent or sibling had a heart attack before 55 (men) or 65 (women).

Frequently asked questions

Seene ka dard gas hai ya dil ka daura — farq kaise pata chale?

Yaqeeni farq ghar par nahi kiya ja sakta, aur yehi sab se bara khatra hai. Gas ka dard aksar khane se taalluq rakhta hai aur dakar se kam ho jata hai; dil ka dard mehnat se barhta hai, aaram se kam hota hai, aur bazu ya jabray tak phailta hai. Lekin dono kaafi milte-julte ho sakte hain — is liye qaida yeh hai: shubah ho to ECG karwayen. Ek ECG ki qeemat ghalat faisle se bohat kam hai.

Kya kam umar mein dil ka daura par sakta hai?

Ji haan. Pakistan mein 30 se 40 saal ki umar mein dil ke daure ab pehle se zyada dekhe ja rahe hain, khas taur par un logon mein jinhen sugar, blood pressure ya smoking ki aadat ho. Umar kam hone ka matlab mehfooz hona nahi hai.

Is a stent or bypass better?

It depends on how many arteries are involved, where the narrowing sits, and whether you have diabetes — not on which sounds more serious. Multi-vessel disease with diabetes often does better with bypass; a single blocked artery in an acute attack is usually stented immediately. Ask your cardiologist to explain the angiogram findings in those terms.
