Munh Ka Cancer in Pakistan: Gutka, Paan, Chaliya and the Warning Signs

Oral cancer is among the most common cancers in Pakistani men, and chewing habits account for the large majority of cases. A sourced guide to early signs, the risk from gutka and paan, and quitting.

Oral cancer is one of the few common cancers with a single dominant, entirely avoidable cause. According to the World Health Organization, oral cancer is most prevalent among Pakistani men of any country in South Asia; it is described in the literature as the second most common cancer in Pakistani women and third most common in men. Chewing habits — paan, gutka, chaliya (areca nut), mainpuri, naswar — together with smoking are held responsible for roughly 70–75% of oral cancers in Pakistan ("Oral cancer: Clinicopathological features and associated risk factors in a high risk population presenting to a major tertiary care center in Pakistan," PLOS ONE / PMC).

At a glance

  • Oral cancer is most prevalent among Pakistani men in all of South Asia (WHO)
  • Chewing habits and tobacco account for an estimated 70–75% of cases here
  • Areca nut (chaliya) is a Group 1 human carcinogen on its own — "paan without tobacco" is not safe
  • Early oral cancer is painless, which is why it is caught late
  • A mouth ulcer that has not healed in three weeks needs examining, not another mouthwash
  • A dentist usually sees it first — this is what a routine dental check is for

Alamat — teen haftay ka qaida

Agar in mein se koi cheez teen haftay se zyada rahe, to daant ke doctor ko dikhayen:

  • Munh mein chaala jo theek na ho raha ho (munh ka chaala jo bhar na raha ho) — sab se aam pehli nishani
  • Munh, zaban ya gaal ke andar safed ya surkh daag (leukoplakia / erythroplakia)
  • Gaal ya zaban par gaanth ya sakht hissa
  • Munh poora na khul pana (munh khulna kam ho jana) — oral submucous fibrosis ki nishani, jo chaliya se hoti hai
  • Nigalne ya chabane mein takleef; awaz ka badalna
  • Zaban ya honth ka sun hona
  • Bay-wajah daant hilna, ya masuray se khoon
  • Gardan mein gaanth

Early oral cancer does not hurt. Pain usually appears once it is advanced — so waiting for pain is waiting too long.

Gutka, paan aur chaliya — asal mein hota kya hai

These products contain around 28 known carcinogens between them. The important and widely misunderstood point is that areca nut is classified as a Group 1 carcinogen by IARC in its own right. "Paan without tobacco" and "sirf chaliya" are not harm-free alternatives — a PubMed-indexed study is titled precisely "Paan without tobacco: an independent risk factor for oral cancer".

Before cancer, chronic chewing typically produces oral submucous fibrosis: the lining of the mouth stiffens, the cheeks become fibrous bands, and the mouth progressively stops opening. It is itself pre-malignant, it is largely irreversible, and it is increasingly seen in teenagers here because gutka and sweetened areca sachets are cheap, sold loose, and marketed like confectionery. Naswar, held in the gum or lip, causes lesions exactly where it sits.

Chhorne ka tarika — quitting

  • Pick a quit date and tell the people you sit and chew with — the habit is social, so the quit has to be too
  • Substitute the ritual, not just the substance: saunf, elaichi, sugar-free gum, roasted chana. Much of the pull is the act of chewing
  • Identify the triggers — after meals, with tea, while driving, at work breaks — and plan a specific replacement for each
  • Expect withdrawal: irritability, poor concentration and cravings for a few weeks. It passes
  • Ask about nicotine replacement if you use tobacco-containing products; a pharmacist or doctor can advise on what is appropriate
  • Get a dental examination now, not after quitting — existing lesions still need checking

Risk falls after stopping, and existing white patches sometimes regress. But mouth opening lost to fibrosis usually does not come back, which is the argument for stopping today rather than at some future point.

Screening and diagnosis

A dentist can examine the whole mouth in a couple of minutes and is the professional most likely to spot an early lesion — including on a routine visit for something else entirely. Suspicious areas are confirmed by biopsy; imaging (CT/MRI) then stages the disease. Treatment is surgery, radiotherapy, chemotherapy or a combination, and outcomes depend heavily on stage at diagnosis.

If you chew anything — paan, gutka, chaliya, naswar, mainpuri — get a mouth examination at least once a year even with no symptoms.

Frequently asked questions

Kya bina tambaku wala paan mehfooz hai?

Nahi. Chaliya (areca nut) khud IARC ke mutabiq Group 1 carcinogen hai — yani tambaku ke baghair bhi cancer ka sabab ban sakti hai. "Sada paan" ya "sirf chaliya" ko mehfooz samajhna sab se aam aur sab se nuqsan-deh ghalat fehmi hai.

Mera munh pehle se kam khulta hai. Kya yeh theek ho sakta hai?

Munh ka kam khulna oral submucous fibrosis ki alamat hai. Agar abhi chaliya/gutka chhor dein to bimari aage barhna ruk sakti hai, aur ilaj se kuch behtari mumkin hai — lekin jo sakhti aa chuki hai woh aksar poori tarah wapas nahi hoti. Isi liye intezar karne ka koi faida nahi.

My ulcer hurts a lot — does that mean it's not cancer?

Painful ulcers are more often benign (aphthous ulcers, a sharp tooth edge, a denture rubbing), and cancer is classically painless early on. But this is a tendency, not a rule, and it is not something to judge at home. The three-week rule stands regardless of pain.

Is sheesha safer than cigarettes for the mouth?

No. A sheesha session delivers smoke over a much longer period than a single cigarette, and shared mouthpieces add an infection risk on top.
