Masuray Se Khoon (Bleeding Gums) in Pakistan: Gum Disease and How To Stop It

Bleeding gums are not normal, and gum disease is the leading cause of adult tooth loss. A guide to gingivitis, periodontitis, scaling, and the links to diabetes, pregnancy and heart disease.

Bleeding when you brush is treated in most Pakistani households as ordinary — a sign you brushed too hard, or that the toothpaste is "strong". It is neither. Healthy gums do not bleed. Bleeding is inflammation, inflammation is gum disease, and gum disease is the leading cause of tooth loss in adults — ahead of decay. The encouraging part is that the early stage is completely reversible, usually within a couple of weeks, with no drilling and no expense beyond a brush and floss.

At a glance

  • Bleeding gums are never normal, at any age
  • Gingivitis (early, gums only) is fully reversible. Periodontitis (bone loss) is not — it can only be halted
  • Gum disease is the leading cause of adult tooth loss
  • It runs both ways with diabetes: each makes the other worse
  • Gum disease in pregnancy is associated with preterm birth and low birth weight
  • Scaling does not loosen teeth or damage enamel — that myth costs people their teeth

Alamat

  • Brush ya miswak karte waqt khoon (masuray se khoon aana) — the earliest and most ignored sign
  • Masuray surkh, sooje hue ya narm (sehatmand masuray gulabi aur sakht hote hain)
  • Munh se badbu jo brush karne ke baad bhi rahe (munh se bu aana)
  • Masuray ka daant se hat jana — daant lambe lagne lagte hain
  • Daanton ke darmiyan naye khaali khaane ban jana
  • Daant hilna (daant hilna) — late, and serious
  • Chabane par dard, ya kaatne ka tareeqa badal jana
  • Masuray aur daant ke darmiyan pip (pip aana)

Gingivitis aur periodontitis mein farq

Gingivitis: Plaque masuray ke kinare par jama hota hai, sozish hoti hai, khoon aata hai. Haddi abhi mehfooz hai. Yeh mukammal tor par theek ho sakta hai — sirf sahih safai se, do haftay mein.

Periodontitis: Agar gingivitis ka ilaj na ho to sozish gehri ho kar daant ko thamne wali haddi tak pohanch jati hai. Haddi ghulne lagti hai, jebein ban jati hain, aur daant dheela hone lagta hai. Jo haddi chali gayi woh wapas nahi aati — ilaj se bimari rok di jati hai, lekin nuqsan palta nahi. Yehi wajah hai ke khoon aane ko nazar-andaz nahi karna chahiye.

Asbab — what causes it

  • Plaque — the soft bacterial film that forms constantly; if not removed daily it hardens into calculus (tartar), which no brush can remove
  • Tambaku, paan, gutka, chaliya, naswar — major contributors; smokers' gums often bleed less despite worse disease, which masks it. See oral cancer
  • Diabetes — poorly controlled sugar worsens gum disease, and gum disease worsens glucose control
  • Pregnancy and hormonal change
  • Vitamin C deficiency; poor nutrition
  • Crowded teeth, broken fillings and ill-fitting dentures that trap plaque
  • Dry mouth, and some medications
  • Stress and immune suppression

Treatment

At home — where most of the work happens

  • Brush twice daily for two full minutes, soft bristles, small circles at 45° to the gum line — gently. Hard scrubbing damages gums and does not clean better
  • Clean between the teeth daily — floss or interdental brushes. A toothbrush reaches about 60% of the tooth surface; the disease starts in the 40% it misses
  • Fluoride toothpaste; change the brush every three months
  • Miswak is a reasonable adjunct with real antibacterial properties — but it does not clean between teeth, so it is not a complete substitute
  • Antiseptic mouthwash for short courses if advised — not as a replacement for brushing
  • Stop tobacco, paan and gutka

At the clinic

Scaling and polishing removes hardened calculus that brushing cannot. For periodontitis, root planing cleans below the gum line, sometimes with local antibiotics, and advanced cases may need flap surgery or grafting. Most people need scaling every six to twelve months.

A dental hygienist does this work and teaches the technique that keeps it from coming back — you can find dental hygienists on DoctorOS, or a dentist for assessment and treatment of advanced disease.

Why it matters beyond the mouth

Gum disease is a chronic bacterial inflammation with a large surface area, and it does not stay local. It is associated with poorer blood sugar control in diabetes, with cardiovascular disease, and — importantly here — with preterm birth and low birth weight in pregnancy. Dental check-ups during pregnancy are safe and worth doing.

Frequently asked questions

Kya daant ki safai (scaling) se daant kamzor ya dheele ho jate hain?

Nahi — aur yeh Pakistan ki sab se nuqsan-deh dental ghalat fehmi hai. Scaling sirf jama shuda calculus hatata hai, daant ya us ki enamel nahi. Kabhi kabhi safai ke baad daant thora hilta mehsoos hota hai kyunki jo calculus usay jakre hue tha woh hat gaya — lekin us calculus ne pehle hi haddi ko nuqsan pohanchaya hota hai. Safai na karwana masla hai, safai karwana nahi.

Masuray se khoon aata hai lekin dard bilkul nahi. Kya intezar kar sakta hoon?

Nahi. Masuray ki bimari aksar bay-dard hoti hai — dard tab aata hai jab bohat der ho chuki ho. Khoon aana hi pehli aur asal alamat hai. Ibtidai marhale (gingivitis) mein yeh do haftay mein poori tarah theek ho sakti hai; agar haddi ghulna shuru ho gayi to phir wapsi nahi hoti.

Is bleeding just from brushing too hard?

Rarely. Healthy gums tolerate normal brushing without bleeding. If they bleed, the usual explanation is inflammation from plaque — and the correct response is to clean that area more carefully and gently, not to avoid it. Bleeding typically stops within one to two weeks of proper cleaning.

Does miswak replace a toothbrush?

It has genuine antibacterial activity and is better than nothing, but it cannot clean between teeth, which is where gum disease begins. Use it alongside brushing and interdental cleaning rather than instead of them.
