Vitamin D Ki Kami in Pakistan: Symptoms, Test and Treatment

Pakistan has the highest vitamin D deficiency prevalence in South Asia in a published meta-analysis. A sourced guide to symptoms, the 25(OH)D test, sunlight, diet and safe supplementation.

Vitamin D deficiency is so common in Pakistan that it has stopped looking like a deficiency and started looking like the norm — which is exactly why it goes untreated for years. A systematic review and meta-analysis of South Asian adults found the highest prevalence in the region was Pakistan's, at 73% (95% CI 63–83%), ahead of Bangladesh, India, Nepal and Sri Lanka ("High prevalence of vitamin D deficiency among the South Asian adults," PMC). Pakistan's own National Nutrition Survey put deficiency at 70% in pregnant women, 66% in non-pregnant women and 40% in children.

At a glance

  • Meta-analysis prevalence in Pakistani adults: 73% — the highest of five South Asian countries studied
  • A separate national estimate found 53.5% deficient and a further 31.2% insufficient — i.e. roughly four in five below optimal
  • Symptoms are vague and easily blamed on something else: tiredness, aching bones, muscle weakness, low mood
  • One blood test (25-hydroxy vitamin D) settles it. Treatment is inexpensive
  • Sunlight through a window does not count — glass blocks the UVB that makes vitamin D

Vitamin D ki kami kyun hoti hai?

Pakistan mein dhoop ki kami nahi hai — phir bhi vitamin D ki kami aam hai. Wajah yeh hai ke jism vitamin D tab banata hai jab khuli jild par seedhi dhoop pare. Ghar ke andar kaam, dhoop se bachne ka amal, poora libas, aur garmi mein dopahar ki dhoop se girez — in sab ka matlab hai ke jild tak UVB pohanchti hi nahi. Gehri rangat wali jild ko utni hi vitamin D banane ke liye zyada waqt chahiye hota hai. Sheeshay ke peechay baithna bekaar hai: sheesha UVB rok deta hai.

Symptoms — what deficiency actually feels like

The frustrating part is that nothing here is specific. Any one symptom has ten other explanations; it is the cluster, and its persistence, that should prompt a test.

  • Persistent tiredness and low energy (thakawat, kamzori) that sleep doesn't fix
  • Aching bones, especially the lower back, hips, pelvis and shins (haddiyon ka dard)
  • Muscle weakness — difficulty climbing stairs or getting up from the floor
  • Muscle cramps and twitching (pathon mein khinchao)
  • Low mood, poor concentration
  • Frequent infections and slow wound healing
  • Hair fall — one of several possible causes, see our guide to hair fall

In children, severe long-standing deficiency causes rickets: bowed legs, delayed walking, a soft skull, and delayed tooth eruption. This is a paediatric emergency in the sense that it is entirely preventable and shouldn't be reaching that stage.

Who is most at risk

  • Women who are pregnant or breastfeeding — and their babies, who inherit the mother's status
  • Anyone who covers fully outdoors or works entirely indoors
  • Older adults, whose skin synthesises less
  • People with darker skin
  • Exclusively breastfed infants without supplementation — breast milk is low in vitamin D
  • People with obesity, coeliac disease, inflammatory bowel disease, or after bariatric surgery
  • People on long-term steroids or some anti-epileptic drugs

The test — and what the number means

The test is serum 25-hydroxy vitamin D, written as 25(OH)D. It is a single blood sample, no fasting needed, and widely available in Pakistani labs. Broadly used thresholds are: deficient below 20 ng/mL, insufficient 20–30 ng/mL, sufficient 30–100 ng/mL. Laboratories differ slightly and some report in nmol/L, so read your own report's reference range rather than comparing to someone else's number.

You do not need to test everyone. Testing is worth it if you have the symptom cluster above, a risk factor, unexplained bone pain, or osteoporosis. Retesting is usually done around three months after starting treatment, not weeks.

Treatment: dhoop, khana aur supplement

Sunlight

Roughly 10–30 minutes of midday sun on arms and legs, several times a week, is the usual advice — longer for darker skin, shorter for very fair skin. Sunlight alone is often not enough to correct an established deficiency, which is what supplements are for.

Diet

Very few foods contain meaningful vitamin D naturally, which is why diet alone rarely fixes a deficiency: oily fish (salmon, mackerel, sardines), egg yolk, liver, and fortified milk or cereal where available. Doodh, anday ki zardi aur machli madad karte hain, lekin sirf khane se kami poori karna mushkil hai.

Supplements

Vitamin D3 (cholecalciferol) is the usual form, sold in Pakistan as sachets, capsules and injections. Dose depends on your level, your age and your other conditions — that is a prescribing decision, not a shop-counter one. High-dose injections are widely handed out here without a test first; that is worth pushing back on, because vitamin D is fat-soluble and genuinely can be overdone. Too much causes high calcium: nausea, excessive thirst, confusion, kidney stones. If you already get kidney stones, mention it before starting.

Calcium and magnesium matter alongside it — vitamin D raises calcium absorption, and correcting D without enough dietary calcium doesn't achieve much for bone.

When to see a doctor

See a general physician if the tiredness and bone pain have lasted more than a few weeks, if a child has bowed legs or delayed walking, or if you have been taking high-dose supplements without ever having a level checked. Bring any previous 25(OH)D reports — the trend matters more than one value.

Frequently asked questions

Kya sirf dhoop se vitamin D ki kami poori ho jati hai?

Halki kami mein dhoop madad karti hai, lekin agar level bohat kam ho to sirf dhoop kaafi nahi hoti — supplement lena parta hai. Aur yaad rahe: sheeshay ke peechay ya poori astin mein baithne se vitamin D nahi banti, kyunki UVB jild tak pohanchti hi nahi.

Kya har mahine vitamin D ka injection lagwana theek hai?

Bina test ke nahi. Vitamin D charbi mein jama hoti hai, is liye zyada matra nuqsan-deh ho sakti hai — calcium barh jata hai, ulti, pyaas aur gurday ki pathri ho sakti hai. Pehle 25(OH)D test karwayen, phir doctor se dose poochen.

How long until I feel better?

Bone pain and muscle weakness often improve over weeks to a few months, not days. If nothing has changed after three months of adequate treatment and a repeat level that is now normal, the vitamin D probably wasn't the whole explanation — that is worth revisiting rather than escalating the dose.

Is vitamin D linked to diabetes and blood pressure?

Deficiency is commonly found alongside diabetes and high blood pressure, but found-alongside is not the same as causes. Correcting vitamin D is worth doing for its own sake; it is not a substitute for treating either condition.
