Fatty Liver (Jigar Par Charbi) in Pakistan: Causes, Tests and Reversal
A meta-analysis put non-alcoholic fatty liver disease at 29.8% of Pakistan's general population — and 58% among people with diabetes. A sourced guide to a condition that is silent, common and reversible.
Fatty liver is the most common liver condition in the country and one almost nobody is told about until an ultrasound done for something else mentions it in passing. A 2024 systematic review and meta-analysis found a pooled non-alcoholic fatty liver disease (NAFLD) prevalence of 29.82% in Pakistan's general population — and far higher in people who already have metabolic disease: 58.47% in diabetes, 74.08% in hypertension and 47.43% in obesity ("Prevalence of nonalcoholic fatty liver disease in Pakistan: a systematic review and meta-analysis," PMC).
At a glance
- Pooled NAFLD prevalence in Pakistan: 29.8% of the general population
- 58% of people with diabetes and 74% of those with hypertension have it
- You do not need to drink alcohol to get it — this is the "non-alcoholic" kind, and it is the common one
- Usually completely silent until it isn't
- It is reversible in the early stages — losing 7–10% of body weight is the single most effective treatment
- No licensed drug reverses it; anything sold as a "liver tonic" is not that drug
Jigar par charbi kya hai?
Jab jigar ke khaliyon mein zaroorat se zyada charbi jama ho jaye to usay fatty liver kehte hain. Aam taur par is ke chaar darjay bataye jate hain:
- Simple fatty liver (steatosis): Sirf charbi, sozish nahi. Yeh sab se aam aur sab se qabil-e-wapsi soorat hai.
- NASH / steatohepatitis: Charbi ke sath sozish aur khaliyon ka nuqsan — ab yeh sanjeeda hai.
- Fibrosis: Nuqsan ki jagah sakht tissue banne lagta hai.
- Cirrhosis: Jigar sikur jata hai aur kaam karna chhor deta hai. Yahan wapsi bohat mushkil hai.
Ultrasound reports commonly say "grade 1, 2 or 3 fatty liver". That grading describes how bright the liver looks on ultrasound — how much fat — not how much scarring there is, which is the thing that actually determines outcome. Grade 1 is not automatically harmless and grade 3 is not automatically cirrhosis.
Symptoms — or the lack of them
Most people have none. When symptoms do appear they are vague: fatigue, a dull ache or fullness under the right ribs (pet ke daayen taraf oopar bhaari pan), and general malaise. By the stage that jaundice (yarqan), swelling of the abdomen or legs, easy bruising or confusion appear, significant damage has already happened. This silence is exactly why it is usually found by accident.
Causes and risk factors
- Obesity, and central obesity in particular
- Insulin resistance and type 2 diabetes
- High triglycerides and low HDL cholesterol
- High blood pressure
- Diets high in refined carbohydrate, sugary drinks and fried food
- Physical inactivity
- PCOS; hypothyroidism; obstructive sleep apnoea
- Rapid weight loss and some medications
South Asians develop fatty liver at lower BMIs than other populations — "lean NAFLD" is well described, so a normal weight does not exclude it.
Tests
- Ultrasound abdomen — the usual first detection, widely available and inexpensive
- LFTs (ALT, AST) — often mildly raised, but frequently normal even with significant disease
- FibroScan / transient elastography — measures stiffness, i.e. fibrosis. This is the test that matters for prognosis
- FIB-4 or NAFLD fibrosis score — calculated from routine blood results and age; a cheap way to decide who needs a FibroScan
- Rule out other causes: hepatitis B and C serology in particular, which are common in Pakistan and change everything about management
- Blood sugar, HbA1c, lipid profile, thyroid
Treatment: charbi kam kaise hoti hai
There is no licensed drug that reverses NAFLD. What works is unglamorous and genuinely effective:
- Weight loss of 7–10% — the best-evidenced intervention. Around 5% reduces liver fat; 7–10% can resolve inflammation and even improve fibrosis
- Cut sugary drinks and fructose — soft drinks and packaged juices are directly implicated
- Reduce refined carbohydrate: white flour, white rice, bakery items. Portion, don't eliminate
- Exercise 150 minutes a week, including resistance work — this reduces liver fat even without weight loss
- Treat the metabolic partners: diabetes, blood pressure, lipids
- Zero alcohol, and caution with unregulated herbal and "liver tonic" preparations, some of which cause genuine liver injury
- Coffee, unsweetened, is associated with lower fibrosis risk in observational studies
Losing weight too fast makes fatty liver worse. Aim for half a kilo a week — a dietitian is useful precisely because the diet needs to be sustainable for years, not weeks.
When to see a doctor
See a gastroenterologist if an ultrasound has reported fatty liver, if LFTs are persistently raised, or if you have diabetes plus abnormal liver tests. The question to ask is not "how much fat?" but "how much fibrosis?" — that is what decides whether this is a lifestyle matter or a liver disease under monitoring.

Frequently asked questions
Grade 1 fatty liver hai — kya yeh khatarnak hai?
Grade 1 aam taur par ibtidai marhala hai aur is marhale par wapsi mumkin hai — yehi is ki achhi khabar hai. Lekin "grade" sirf charbi ki miqdar batata hai, jigar mein sakhti (fibrosis) ki nahi. Is liye behtar sawal yeh hai: kya fibrosis hai? Us ke liye FibroScan ya FIB-4 score karwayen.
Kya koi dawai hai jo jigar ki charbi khatam kar de?
Filhal koi manzoor shuda dawai nahi hai jo fatty liver ko palat de. Bazaar mein bikne wale "liver tonic" is ka mutabadil nahi — aur kuch ghair-mustanad herbal cheezein khud jigar ko nuqsan pohanchati hain. Asal ilaj wazan kam karna, meethay mashroobat chhorna aur warzish hai.
I don't drink alcohol. How do I have liver disease?
That is exactly what "non-alcoholic" means. The common driver in Pakistan is metabolic — insulin resistance, central weight, refined carbohydrate and inactivity — not alcohol. It is now the leading cause of chronic liver disease in much of the world.
Can fatty liver turn into cirrhosis?
A minority of cases progress through inflammation to fibrosis and eventually cirrhosis, typically over years to decades. Progression is much more likely with uncontrolled diabetes and continued weight gain — which is the practical reason to act at the "harmless" stage.
